Client Preparation Questions for Supervisors/Managers and DHRMs

Employee Relations can expedite your case if you have the following prepared before contacting us:

  1. What is the employee's name?
  2. What is the employee's department?
  3. What is the employee's payroll title?
  4. What is the employee's hire-date?
  5. What is the name of the employee's supervisor?
  6. Is the employee covered by a union (name of union) or Personnel Policies for Staff Members (PPSM)?
  7. Is the employee a career employee? Is the employee on probation?
  8. Is the employee a limited employee?
  9. Is the employee a contract employee?
  10. What is the current issue or question or incident?
  11. If applicable: Has the employee been coached about the issue before? If so, when and by whom?
  12. Does the employee have counseling or discipline or performance reviews in the personnel file? If so, what specifically and what are the dates?
  13. If applicable: List policy, contract, Central HR resource, or chapter from the Guide to Managing Human Resources that you reviewed:
  14. Any other issues, concerns or facts that would be helpful to know?
